The first error was a failure to laminate the plys of the girder. Two 2x members 2-1/2" apart are not a girder. The girder span charts are based on the plys being laminated according to the prescription connection detail. Secondly, girders are required to have bearing for the full width (1-1/2"). There is no bearing at all here; the contractor relied on the shear strength of the screws.
